Medtronic Diabetes has notified the Pipeline Industry Benefit Fund that they are voluntarily recalling Quick-set infusion sets that have lot numbers starting with the number "8" ("Lot 8" Quick-set infusion sets.) These infusion sets are used with MiniMed Paradigm insulin pumps. No other Medtronic infusion sets are affected. (Some of our members may receive a product recall notification direct from Medtronic Diabetes instructing them to immediately discontinue use of the "Lot 8" Quick-set infusion sets.)
Medtronic is taking this action because some of the infusion sets do not allow the insulin pump to vent properly, which is necessary to equalize the pressure in the reservoir compartment with the surrounding atmosphere. Improper venting could potentially result in too much or too little insulin being delivered and may lead to serious injury. Medtronic has corrected the product issue and will be providing members with replacement Quick-set infusion sets at no additional charge. To address patient concerns, Medtronic has set up a website at www.medtronicdiabetes.com/lot8 to answer questions and automate the process of initiating infusion set replacements. The website will be the quickest and most efficient method for exchanging product.
The PIBF Claims Department is receiving claims under a member's "nickname" instead of their given or proper name. This delays processing of the claim because the names do not match the Blue Cross Blue Shield or PIBF internal software systems that are programmed to recognize the participant's proper name. Every member should have completed a Census Card that the Fund has on file - please use your proper name as provided on this card.
The PIBF would like to remind members that the deadline for submission of HRA claims for the year 2009 is March 31, 2010. Submitting claims in a timely manner will greatly help the Claims Department and prevent a "bottleneck" in the processing of these claims.
The PIBF has requested that CVS send out a "satisfaction questionnaire" to all members using their mail order prescription service. This questionnaire should be mailed to members in the next 60 days. If you are using the mail order prescription service, please complete and return the questionnaire as instructed.
